Categories of personally identifiable information collected by the Website include: names, email addresses, phone numbers, home or business addresses, and personal or medical information disclosed by the user on the Website. The personally identifiable information requested by us is collected for purposes of scheduling treatments, providing health-related information, responding to health or treatment-related inquires, marketing, and billing and payment inquiries.

 

Additionally, the Website or its hosting server collects or may collect, at least temporarily, the following information: Internet Protocol (IP) address of the computer or device being used, web pages requested, referring web page, browser used, operating system used, and date and time of visit. We do not knowingly collect personally identifiable information from minors. If APMD is made aware of collecting information from a minor we will delete this information.
